New Patients​

We have an easy to use patient portal through NextGen where our patients have access to all medical information, labs, appointment schedule and can pay their bills!

If you have questions about the practice please email Dr. Lisa at drlisa@lisajlmd.com. The wait for a new patient visit is 6 months or more but sometimes sooner depending on your availability.

TO BEGIN:
1. fill out the “portal access request” to be on the waiting list.

You should receive an email link once you have been registered for the practice to set up your online portal + gain access to the schedule where you can book appointments. You will be registered for the practice once your name comes up on the waiting list. The email will come from: HFAlerts@nextgen.com & the subject line will be “Activate your YourHealthFile Patient Portal account.” Check your spam folder if you don't receive this email in your regular email folder.
2. Follow the link to begin the set up process. You will be prompted with a series of patient information questions. An. It is recommended to use a laptop or desktop computer (not a mobile phone).

Note: The Patient Portal is best accessed using Mozilla Firefox as your browser. The Patient Portal provides a private HIPPA compliant chart and communication portal. Emails submitted from this website or through the portal are never shared or sold.
3. Print and fill out an intake form and bring it to your first appointment with Dr. Lisa. This is required for all first time visits. Forms can be accessed here:
